The Overlooked Digestive Player Behind Your Bloating: Why Bile Deserves Your Attention

Kristin had spent years dialing in her wellness routine in an attempt to mitigate her chronic bloating and to feel strong, confident, and healthy. When we started working together, she had her routines locked in: eating whole foods, supporting her minerals through electrolytes and supplementation, spending time outside daily, sleeping 8 hours a night. But despite her efforts, she was still feeling bloated every day. When we ran her labs, they showed low vitamin D, poor fat digestion, and bacterial overgrowth in her gut.

So where did we start in supporting her? The obvious starting points may be vitamin D supplementation, digestive enzymes, and a "kill protocol" to address her bacterial imbalances. We eventually do all of those things, but we didn’t start there. We started with bile support.

Many people associate bile with digesting fats, as that is one of its central jobs, but bile also plays a critical role in detoxification, hormone regulation, microbial balance, and fat-soluble vitamin absorption. So let's go through these one by one, and it will become clearer to you why this was such a critical place to start.

Fat digestion

Fat is hydrophobic, meaning that it doesn't mix with water. This can be a problem because your digestive tract is a watery environment. So without help, the fat from your dinner would just become one stagnant, greasy blob, completely inaccessible to the enzymes that are supposed to break it down.

That's where bile comes in. Bile acts like a detergent, similar to what breaks up grease on a dirty dish. It surrounds fat molecules and breaks them into much smaller droplets through a process called emulsification. This gives your digestive enzymes more surface area to work with, so they can help break down your food.

Once your fats are broken down into fatty acids and other byproducts, bile does one more thing: it packages them into tiny transport vehicles called micelles. Picture a micelle as a little taxi. The fat-loving parts of bile face inward, wrapping around the fat, while the water-loving parts face outward, so the whole package can travel through the watery lining of your gut and get delivered to your intestinal cells for absorption.

If bile production or flow is low, this process doesn’t work as intended. Fat isn't properly broken down, digestive enzymes can't access it efficiently, and a lot of that fat ends up moving through your gut undigested, which is a common contributor to bloating, especially after fattier meals.

Detoxification & hormone regulation

Bile isn't just involved in breaking down what you eat; it's also one of your body's main routes for getting rid of things it no longer needs, including excess hormones.

For example, after your liver processes estrogen and prepares it for removal, a large portion of that estrogen gets packaged into bile and sent into your intestines for elimination (meaning that you can poop it out). 

When bile flow is sluggish, the whole process slows down. Estrogen that was supposed to exit via the gut lingers longer, has more opportunity to be reabsorbed, and can contribute to symptoms associated with estrogen dominance, such as heavier or more painful periods, breast tenderness, mood swings, and worsened PMS. This is a big reminder that "detox" is really all about making sure the elimination routes you already have, like bile flow, are working as intended.

Microbial balance

Your small intestine is supposed to have relatively few bacteria compared to your colon, and bile helps to ensure it stays this way. Bile acids are directly toxic to many bacteria and fungi because they disrupt microbial cell membranes and make it hard for bacteria to establish colonies where they shouldn't be. On top of that antimicrobial action, bile acids activate signaling pathways that trigger your gut lining to produce its own antimicrobial defenses, adding a second layer of protection.

When bile flow is reduced, that protection weakens. Research in both animal models and humans has shown that conditions that lower bile acid delivery to the small intestine, like bile duct obstruction or impaired liver function, lead directly to small intestinal bacterial overgrowth, or SIBO, and restoring bile acid delivery has been shown to reverse it. 

Antimicrobial treatment (a "kill protocol") can clear an overgrowth, but if bile flow was the reason your small intestine's defenses were weakened in the first place, bile flow needs to be supported to ensure the overgrowth doesn’t come back. That's a big part of why SIBO so often recurs after treatment, and why bile support has to be part of a long-term solution when addressing SIBO.

Fat-soluble vitamin absorption

Remember those micelle "taxis" from the fat digestion section? They're not just carrying fatty acids, they're also the only way your fat-soluble vitamins, A, D, E, and K, can get absorbed.

These vitamins are fat-soluble, meaning they need to travel inside a fat-containing package to cross into your intestinal cells. Without adequate bile, these vitamins can pass straight through your digestive system, undigested and unabsorbed, even if you're eating plenty of them or taking a high-quality supplement.

This is exactly what we saw with Kristin: her vitamin D was low because her body wasn't producing enough bile to actually absorb it, despite her taking a high-quality vitamin D supplement. It's a good reminder that when our digestive system isn’t functioning optimally, we may not be getting all of the nutrients our body needs despite eating a nutrient-rich diet.

Signs you might have low bile

Low bile symptoms can stem from low bile production and/or impaired bile flow, and they tend to show up in ways that are easy to blame on other things. Some signs to look out for include:

  • Chronic bloating, especially after meals

  • Right shoulder pain

  • Fatigue after eating, especially fattier meals

  • Constipation

  • Pale or clay-colored stools

  • Floating or greasy stools

  • Hormonal imbalances, including estrogen dominance symptoms like heavy or painful periods, breast tenderness, and worsened PMS

  • Dry skin or dry eyes

  • Poor tolerance to fatty foods

  • Low vitamin D despite adequate intake or supplementation

How to support bile production and flow

To support bile production, start meals with apple cider vinegar, lemon water, or digestive bitters, all of which can help stimulate your body's natural bile-making process. If you suspect low stomach acid is also part of the picture (read more about stomach acid in this post), consider adding betaine HCl with protein-rich meals.

There are also specific nutrients your liver needs as raw materials to make bile. Choline, found in egg yolks, helps your body produce phosphatidylcholine, a compound that keeps bile fluid and functional. Glycine, found in collagen and bone broth, is one of the amino acids your liver uses to conjugate bile acids into their active, usable form.

To support bile flow, castor oil packs over the liver a few times per week is a beneficial tool. Staying well hydrated with water and electrolytes also helps keep bile thin enough to flow easily, and getting enough soluble fiber (think flax, beans, and oats) helps bind up used bile and hormone metabolites so they can actually leave your body instead of recirculating.

Finally, don't forget the piece that's easy to overlook: your body only makes bile in response to fat. Each meal should include a real source of fat, like ghee, avocado, salmon, chicken thighs, or olive oil, to actually give your gallbladder a reason to release the bile you're working so hard to support.
